For Wellmark BCBS Members
- We will bill your insurance for all covered services, which will include your standard co-pay
- Because ketamine is considered a non-covered treatment, there is an additional out-of-pocket fee of $125 per ketamine session
Self-Pay Rates
- Initial Ketamine Intake: $225
- Ketamine Session: $350 per session
A typical series includes 6 treatments plus the initial evaluation = $2,325 - A sliding scale fee is available upon request for those with financial need

- CareCredit
Important Notes
- We do not accept Medicaid or Medicare.
- Payment is due at the time of service.
